Care Coordinator
RelyMD is a growing digital health company that provides telehealth solutions to health systems, managed care organizations, and employers.  Founded by a group of innovative emergency medicine physicians and healthcare entrepreneurs, we’re on a mission to foster healthier communities and workplaces by delivering reliable, trusted medical care – anytime, anywhere.  The Care Coordinator is responsible for assisting our telehealth providers with patient intake, assigning patients to physicians, and assisting patients with discharge and customer support needs.

What you’ll be doing:

  • Assist with patient intake by capturing required demographics, health information, and current condition using our proprietary telehealth software.

  • Monitor patients as they enter our virtual waiting rooms to ensure they receive prompt care from our provider network while maintaining appropriate expectations.

  • Assign patients to the most appropriate providers based on medical condition and physical location while communicating patient queue updates to all available providers.

  • Ensure patients receive clinical summaries, work notes, discharge instructions, and prescriptions as necessary.

The ideal candidate:

  • Medical experience required (Medical Assistant or LPN preferred)

  • Available to work 12-hour shifts which may require nights and weekends as needed

  • Position is remote, you'll need reliable internet and a quiet work space

  • Must be efficient on computer programs (Experience in Microsoft Word, Excel, etc.) and a fast learner

  • Must be able to communicate with physicians on professional level

  • Must make customer service a top priority
